Driving licence check for drivers

Q: Where are the details on the cards?

On the EU driving licence, the expiry date is in field 4b, the categories in field 9, each category's validity in field 11 and code 95 with its date in field 12. Some countries print code 95 on a separate certificate. On the driver card, the number is in field 5b and the expiry date in field 4b.

Q: Do the recipients need to log in?

No. They fill in the form via a link on a phone or computer, with no login.

Q: Where can we see the answers?

Under Submissions in NextSign, where you can also download uploaded files. Sensitive answers, such as ID numbers, are only shown when you ask for them. The answers are deleted automatically after the period set in the form's settings.

Q: Can we change the template?

Yes. The template becomes an ordinary form in your account. You can change its questions and texts before you send.

Licence, code 95, driver card and ADR from every driver

If your drivers drive lorries or buses, there is more to keep track of than the licence: the validity of categories C and D, code 95, the tachograph driver card and the ADR certificate. The template collects it all in one form.

Drivers only answer what applies to them. The C and D dates only appear for the categories they tick, and the ADR date only for drivers with an ADR certificate. Photos of the documents can be uploaded but are optional.

Repeat the check, for example once a year and when you hire a new driver.
